If you are transferring your service to a different location, you will need to provide the 911 address of the new location and a copy of a lease for that property if you are renting.
If you are disconnecting your service, the account holder (the person whose name is on the bill) is required to make the request. Once it has been disconnected a final bill will be issued and applied to the original security deposit that was paid. If the final bill is more than the security deposit, you will receive a bill for that amount which will need to be paid in a timely manner. If the final bill is less than the original deposit, you will receive a refund check within 30 days of the disconnect date.
